By providing your credit card information and signing this authorization form, you agree to the following terms and conditions:

1. Authorization for Payment:

   • – You authorize Simple Moving, Inc. to charge your credit card for the total cost of your move as outlined in your confirmed moving quote and/or moving confirmation.
   • – Charges may include, but are not limited to, hourly rates, double drive time, additional labor, packing materials, extra heavy items, and any other applicable fees as described in your quote.

2. Pricing Confirmation:

   • – The final amount charged will reflect the services provided based on the agreed-upon hourly rates, and additional services detailed in your moving quote and confirmation.
   • – Any changes to the scope of work, such as additional items, extra labor, or delays not caused by Simple Moving, Inc., may result in additional charges.

3. Cancellation and Rescheduling Policy:

   • – Cancellations or reschedules made within 24 hours of the scheduled time may be subject to a cancellation fee equivalent to a three-hour minimum charge.

4. Disputed Charges:

   • – In the event of a dispute, you agree to first contact Simple Moving, Inc. directly to resolve the matter. Filing a chargeback without first allowing Simple Moving, Inc. to address your concerns may result in additional fees or legal action.

5. Damage Claims and Liability:

   • – Simple Moving, Inc. follows industry-standard procedures for handling and protecting your belongings. Any claims for damages must be submitted inwriting within the timeframe specified in your moving contract.
   • – Simple Moving, Inc.’s liability for damages is limited to $0.60 per pound per damaged item. If you require full value protection for your belongings, it is your responsibility to purchase additional coverage.

6. Cardholder Responsibility:

   • – You confirm that you are the authorized cardholder or have permission from the cardholder to authorize this transaction.
   • – You understand that by submitting this form, you waive any rights to dispute the charges related to the agreed-upon services unless fraud or unauthorized use has occurred.

7. Confidentiality and Security:

   • – Your credit card information will be securely stored and used only for authorized charges as agreed in this form. Simple Moving, Inc. complies with PCI standards to ensure the safety of your personal data.

8. Deposit and Final Payment:

   • – A deposit may be charged upon scheduling your move. The remaining balance will be processed on the day of your move once services are completed.
